1. Copy of your current mortgage statement

2. Copy of your homeowner's Insurance ... coverage page

3. Two (2) years of year-end W-2 statements (If self-employed, then one (1) year of bank statements).

4. Two (2) current pay check stubs

5. One (1) current bank statement
